Member, steward or other authorized caller, plus preferred callback information.
Worksite & time
Employer, location, department, shift, date and approximate time.
What happened
A plain-language account of the event or condition being reported.
Who was involved
Names or roles of people involved, if the caller is able and authorized to provide them.
Immediate concern
Whether the caller identifies an urgent safety, discipline or representation concern requiring quick union follow-up.
Documents / witnesses
Note whether there are documents, photos, witness names or other information for the union to request later.
